Permit applications for certain connections to the South and Center Chautauqua Lake Sewer Districts can be found here. If you are not sure what permit you need, please call the South and Center Chautauqua Lake Sewer Districts at 716-664-9727.
- Building Sewer Permit Application (PDF) - This permit application is for all users for new construction, repairs, and disconnects. Print out application, fill out, and mail or deliver to SCCLSD along with drawings and payment as described on application.
- Grease Interceptor Permit Application (PDF) - This permit is for those who are preparing to install an outdoor, in-ground grease interceptor. Print out application, fill out, and mail or deliver to SCCLSD along with drawings as described on application. There is no associated fee for this application.
- Significant Industrial User Permit Application (PDF) - This permit application is for industrial users only. This application must be completed in addition to a connection permit. You can print out the forms, fill them out, and mail them, along with drawings and payment as described.
- SIU Permit Renewal (PDF) - This is the renewal application for industrial users only.
- Capping Permit (PDF) - Use a regular permit.
